What is Vitiligo?

Vitiligo is a long-term condition where patches of skin lose their pigment, resulting in lighter areas appearing on various parts of the body. This occurs when the cells responsible for skin colour, called melanocytes, are destroyed, often due to the immune system mistakenly attacking these cells.

Causes, Triggers, and Symptoms

While the exact cause remains uncertain, several factors may trigger or worsen the condition, including genetics, severe sunburn, stress, skin injuries, and exposure to certain chemicals. Vitiligo is also commonly linked to other autoimmune conditions such as thyroid disorders, type 1 diabetes, and lupus, indicating a higher risk for individuals with these coexisting conditions.

The primary symptom is the appearance of lighter patches on the skin, often developing in sun-exposed areas such as the face, hands, arms, scalp, and around the eyes. These patches may spread to other parts of the body over time. Some individuals may also experience early greying of hair in affected areas.

Treatment Options for Vitiligo

There are several treatment options available for managing vitiligo based on the extent and location of the depigmented patches. Here are some of the most common treatments:

Topical Treatments

Dermatologists may prescribe corticosteroids or immunomodulators to reduce inflammation and help slow down the spread of vitiligo. These creams are applied directly to the affected areas, and although they may take several months to show noticeable results, they can sometimes restore some of the lost pigment. The effectiveness of these treatments varies depending on the location and severity of the vitiligo.  There is a new novel effective topical treatment, ruxolitinib which was approved recently for vitiligo.

Phototherapy

This treatment involves exposing the skin to controlled ultraviolet (UV) light, particularly narrowband UVB, which can stimulate repigmentation in areas affected by vitiligo. Phototherapy is usually done in a clinic with medical supervision and may need multiple sessions over several weeks or months for optimal results. It is often recommended for larger or more widespread patches.  Home phototherapy devices may be suitable for treating small areas, thereby minimizing the need to travel to a dermatology center. It is advisable to acquire these devices through a medical professional who can provide guidance and monitor progress effectively.

Oral Medication

In some cases, oral medications may be prescribed to manage vitiligo, particularly if the condition is extensive or other treatments have not been sufficient. These medications are aimed at modulating the immune response and encouraging repigmentation. A dermatologist will assess each patient individually to determine if oral therapy is suitable.

When to Consult a Dermatologist

If you notice light patches or changes in skin pigmentation, consult a dermatologist for an accurate diagnosis. Early consultation is recommended, especially if patches are spreading, if the condition affects your quality of life, or if you have a family history of autoimmune conditions. A dermatologist can provide appropriate treatment options, monitor progress, and screen for related autoimmune diseases, helping to manage vitiligo effectively.

Conclusion

Vitiligo is a manageable condition, but early intervention and professional guidance are key to achieving the best outcomes. Consulting with a dermatologist can help you explore effective treatment options tailored to your needs, ensuring the best possible care for your skin.
